1. To add an assessment to a child record, choose Child Administration > Search Child.
2. When the Child–Search and Manage page opens, use the Search features to locate the child record.

4. When the Assessments page opens, click .
5. The New Complete Assessment page provides options for entering assessment information. Raw Scores or Item Details—Select one of these options to indicate the type of scores you are entering for the assessment. Note: If you enter the raw scores, you will be responsible for the basal and ceiling for each score. Not all reports are available using this method. Select Domain—Click the check boxes to identify the domains that were evaluated during the assessment. Click the All check box to select all of the domains.
Test Date—Type the test date in the corresponding text box for each domain that was assessed. To quickly add both domain and test date information, click and select the date from the pop-up calendar. Select the domain(s) assessed and click to add your selections to the child record. Examiner—Type the examiner’s name for each domain assessed or click and select the name and the domains from the lists. Click the button to add your selections to the child record. Spanish—If the assessments were administered in Spanish, click the corresponding check boxes to indicate the procedure (Structured [S] or Interview/Observation [I/O]) used to administer the assessment for each domain.
6. Enter the scores. If you selected to enter raw scores, type the scores for each domain in the appropriate text box. If you selected to enter item details, click and select options to indicate the score and procedure for each item that was administered. Click to add additional information about an item. Click to view the basal and ceiling for the current domain. Click to proceed to the next domain or to skip a domain. After you have completed entering scores, the program lists the age-equivalent (AE), percentile rank (PR), and standard scores (SS) for each domain that was assessed. Click to view complete totals for each assessed domain.
7. Click to add validity information and/or notes about the child and the test session. Click to add the information to the child record.
8. Click to add a Program Note to the assessment. Click to add the information to the child record.
9. If you entered raw scores, click to save the new complete assessment to the child record. If you entered item details, click .
