Adding Notes to a Child Record

There are three ways to add notes to a child record. You can add Program Notes directly to an assessment, notes to an item in an assessment, or Head Start notes to the child record.

 

       Adding Program Notes to an Assessment

       1. To add a note directly to an assessment, choose Child Administration > Search Child.

                        When the Child–Search and Manage page opens, use the Search features to locate the child record. Click

               2. When the Assessment page opens, click

_img123 to open the Program Note box.

 

               3. Type in the top box to enter a free-format note or click

_img124 to select a predefined Program Note.

 

4.Click

_img120 to update the child record.

 

Adding Notes to an Item in an Assessment

1. To add a note directly to an item in an assessment, choose Child Administration > Search Child.

       When the Child–Search and Manage page opens, use the Search features to locate the child record. Click

2. When the Assessments page opens, click the date to open the assessment.

 

3. Click Item Details and select the domain or subdomain to which you want to add the note. Scroll to the bottom of the page and click

4. When the Edit page opens, locate the item and click

_img127. Type the information you want to add to the assessment and click

_img129 to update the child record.

Note: A score must be entered for the item before the program saves the note.

 

Adding a Head Start Note to a Child Record

1. To add a Head Start note to a child record, choose Child Administration > Search Child.

       When the Child–Search and Manage page opens, use the Searchfeatures to locate the child record. Click

2. When the Notes page opens, click

3. Enter the examiner’s name.

_img132 Tip: To search for an examiner, click

_img133 and locate the examiner’s name from the list or enter First Name, Last Name, and/or Site Name information and click

_img134. To select a name from the list, click the

radio button and then click

4. The program automatically enters the current date. To change the date, type a different date in the Note Date text box or click

_img136 and select the date from the pop-up calendar.

 

5. Select a domain from the list and type the note in the text box. Click

_img137 to save the note.
